CONTESTS AND APPEALS
Individuals may contest the contents or appeal the entries in the
DA Form 3479-R. Complaints shall be directed through channels to the
Commander, USAAVNC, ATTN: ATZQ-ATC-MO, Fort Rucker, AL 36362-5265.
5.
RETENTION
These records are permanent and shall remain active while an
individual is an air traffic controller.
They will be transferred
with the Military Personnel Records Jacket, US Army (MPRJ) or
official civilian personnel folder on termination of active service.
The records of a reclassified controller shall be returned to the
individual when all reclassification actions are complete.

DA Form 3479-1-R

TRAINEE OR CONTROLLER EVALUATION
DA Form 3479-1-R provides a written evaluation of an individual's
training progress or job performance.
It also provides a means to
correct weak points.
