TASK DESCRIPTION
In this lesson you will learn to maintain training records and forms.
LEARNING OBJECTIVE
TASK:
In this lesson, you will be able to conduct facility
training and maintain and record information on facility
training on each controller assigned to your facility.
CONDITION:
While serving as the air traffic control (ATC) facility
training supervisor and given preselected situations,
training records, and forms.
STANDARDS:
According to AR 95-2 and TC 95-93, the training
supervisor must maintain the facility training records
and forms for each assigned controller performing ATC
duties in a facility.

INTRODUCTION
Proper preparation and maintenance of forms and records are essential
to accurately show the status of ATC training.
While some entry
requirements are precise, other entries are left to the discretion of
the records custodian.
Each ATC facility shall maintain a complete and current DA Form 3479-
R (Training and Proficiency Record--Air Traffic Controller) for all
military and civilian controllers.

document all ratings and proficiency, qualification,
