PREPARATION

a. Preparation of DA Form 3479-1-R is self-explanatory. The form

is used to record trainee or controller proficiency and qualification

at all control positions.

The evaluator is the ATC or facility

chief, air traffic control specialist (ATCS) or control tower

operator (CTO) examiner, training or shift supervisor, or trainer.

The

evaluator

shall

make

the

appropriate

entries

(SAT

for

satisfactory or UNSAT for unsatisfactory), make the necessary

comments, and sign the form.

These are important forms; therefore,

they must be completed properly and on time.

b. The trainee or controller receiving the evaluation may agree

or disagree with the evaluation and make any comments he feels

necessary.

Reviewing this document ensures that the form is filled

out properly, that the individual makes the necessary comments, and

that he signs and dates the form. The reviewing authority shall be

the ATC or facility chief or the training supervisor.

After the

evaluation is signed by the reviewing authority, the facility chief

may require that the shift supervisor, trainer, or trainee initial

the DA Form 3479-1-R.

performance of a trainee or controller at more than one control
