(4) When rerecordings or transcripts are not requested within
six months, tapes will then be returned to service.
All relevant
After three years, the
Commander, United States Army Aviation Center, ATTN:
ATZQ-ATC-MO,
Fort Rucker, AL 36362-5265, will be contacted for release of these
tapes.
(5) When the original recorded tapes are to be made part of an
accident or incident investigation file, only the part that includes
the conversation that took place during the time of the particular
incident and an initial and terminal overrun of five minutes of the
tape's running time must be retained. After the pertinent portion of
the tape has been removed, the remaining tape will be spliced and
returned to service.
(6) When transcripts, rerecordings, or copies of tapes are
required, all data that is pertinent to the incident from at least
five minutes before the initial contact to at least five minutes
after the last contact will be included.
To rerecord, a direct
electronic connection between the playback and the recording
equipment will be used whenever possible. Personnel supervising the
reproduction process shall preface the copy with a statement
containing the information furnished at the beginning of the written
transcriptions and shown in (8) through (12) below.
If several
channels are recorded, separate portions of the copy will also be
prefaced by a statement containing the information normally furnished
at the beginning of written transcriptions.
The statement shall
include the date and time of reproduction, the name of the person
supervising
the
reproduction,
and
a
certification
that
the
reproduction is a true copy of the original recording. Handling and
playing back the original recording must be kept to an absolute
minimum.
(7) When rerecordings of transcripts are performed and
certified, they are considered official records and shall be retained
according to AR 25-400-2 and TC 95-93. They are treated the same as
original tapes or records. If written authorization is received from
the airfield commander or investigating team, tapes that have been
rerecorded or transcripted may be returned to service immediately.
If written authorization is not received, the tape will be retained
for six months.
(8) DA letterhead will be used as the first page of each
transcript.
The information listed in the Figure shall be included
on the first page.
(9) The completed transcript will be marked FOR OFFICIAL USE
ONLY (SPECIAL HANDLING REQUIRED).
